How membership works

Give online. Make your annual gift through our secure Bluevine page.
Send your details. Fill in the form so we know where to mail your certificate and pin.
We verify and mail. After your payment clears, we mail your certificate and pin within 2–3 weeks and email your donation acknowledgment.

Membership supports our 501(c)(3) mission. It is not insurance, a discount plan, or a clinical benefit, and it does not change how patients are seen at our clinic.
